We never want costs to stand in the way of quality treatment, which is why we are dedicated to providing our patients with care that is both effective and affordable. Your treatment plan will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. We accept payment by cash, checks, and Visa, MasterCard, Discover and American Express credit cards. We offer flexible in-house payment plans and financing through CareCredit.  We also offer discounts for payment in full with cash or check and for multiple family members in treatment.

If you have any questions about financing or payment, ask us! We will thoroughly explain your choices and work hard to accommodate you.

Insurance

When you first visit our office, bring your current insurance card with you or provide the name, address and telephone number of your insurance carrier. If you change insurance companies or join another employer’s plan, please be sure to let us know.

As a convenience to you, we will file your insurance claims for you and accept insurance assignment for charges pertaining to care provided by our office.

Our primary financial relationship is with our patients and their families and not with their respective insurance companies.  Financial arrangements can be made based on your estimated insurance benefit; however, any outstanding insurance claims not paid are the responsibility of the patient or the patient's family.

What is Covered in the Cost of Treatment?

Your orthodontic investment includes all office visits and procedures performed by Dr. Smiley during the estimated length of orthodontic care. Our office will provide all appliances needed to complete the plan of treatment, including the initial set of retainers, 12 months of monitored retention care and final records at the end of treatment.

What is Not Covered?

  • Late Payments – If payment is not received there will be a $15.00 late fee charged after a 10 day grace period and it will be applied to the account on the 30th of the month.
  • Missed Appointment Fees – There will be a $45.00 missed appointment fee charged after two appointments are missed without notification.
  • Repairs – There will be a per visit repair fee for orthodontic repair appointments needed beyond the three that are provided at no additional cost.
  • Lost/Broken Appliances – lost or broken retainers or appliances will result in additional charges to your account.
  • Treatment Extension – If treatment time extends beyond the original estimated treatment time due to poor cooperation, there may be an extended monthly treatment fee of $100.00 charged until treatment can be completed.
  • Retention Visits – There will be a $35.00 visit charge for all retention visits after the first year of supervised retention.
  • Other – Charges for general dentistry procedures, dental cleanings, routine digital X-rays, extractions, periodontal work, surgical exposure(s) and jaw surgery are NOT included in the cost of your treatment.
